A. Titan will happily accept returns, repair, and warranty items upon request using the online form below. Returns for unwanted products that have not been sterilized and/or used, and shipped from our warehouse may under go an evaluation and receive a refund, if the product falls within a 30 day window of when it was received. After 30 days a house credit will be issued onto the account. Should a distribution partner want to return items, please contact us directly as we honor returns up to 6 months at a 15% restocking fee.
Repairs require a $25 processing fee that covers both inbound and return shipping costs associated with each repair and is charged when the form is submitted. Once your repair or return form is sent, you'll be redirected to page with instructions on creating your UPS shipping label. Please select the appropriate option below to get started:

Please note: Warranty does not apply to normal wear and tear and for use of instruments beyond limitation. Warranty does not cover instruments that have been altered or sharpened in any way.
All A. Titan products are warranted to be free from defects in workmanship and materials. Any product which proves to be defective will be repaired or replaced (at our option) free of charge. Tungsten-carbide instruments (needle holders) are guaranteed for the first three years of normal use. Any modification or alteration made to our instruments other than by A. Titan Instruments will render the warranty null and void. All warranty claims must be made directly with A. Titan Instruments. Any and all returned to the distributor and NOT to A. Titan will render the warranty null and void. Warranty does not cover shipping and handling. Above policies subject to change without notice.
